De Anza Youth Soccer League (DYSL)

 

All adults 18 years of age and over who are rostered on any Cal North affiliated team must be Live Scanned.  The Live Scan is conducted only once on an individual within the Cal North organization and follows the individual within the organization as long as the individual is a volunteer for Cal North. Minors rostered to a team as staff members will not be required to obtain a Live Scan. 

 

Step 1)  Register in Cal North’s Data Management System

Register in Cal North's Data Management System:  deanazyouth.calnorthlive.com/ coaches.  Make sure you print your "Request for Live Scan form".  We are District 2, League 18, Club 1, "CDL" stands for California Driver's License.

 

Step 2)  Visit a Capital Live Scan OfficeYou can find a list of Capital Live Scan locations on their website HERE.

                                                                            

What to Bring

(1)  A current valid California Driver's License or California ID Card.  Also accepted are passports, out of state Driver's Licenses, military ID cards, ID from the Mexican Consulate, and  

(2) a completed Capital Live Scan request form (printed in Step 1 above).  

 

Payment

There should be no charge for this service as Cal North will bill DYSL for the charges.
